All athletic paperwork is due before a student-athlete can participate in any sport or athletic event. This includes the EL2 (physical form), the EL3 (release form) and the school’s consent form. This is for middle and high school only. Lower school students DO NOT need the EL2 and EL3 forms, only the consent form.
